99E - There is one invoice dated 1-17-17 from AG pest and there is no evidence of current infestation. The facility kitchen floor has been cleaned well. The built up grease is mostly removed from under the grill. Keep working on this as there is still a small amount of build up in the center under the grill in hard to reach area. I did not observed any rodent feces. I did not observed any trapped cockroaches on existing sticky traps. There are still some traps set and active pest management is still in use. Good. Keep working on pest prevention by cleaning food debris from the floor and behind equipment every day and monitoring for evidence of pests. Remove dead ones right away when you see them. Impliment increased pest control as needed in the future. 0

6-501.111(C) - During this re-inspection I observed mouse droppings still present by the steps, and under dry storage racks. I observed food and grease build up still under and behind all equipment that was instructed to be cleaned up in the last re-inspection to limit harborage of pests. There were multiple kinds of pest traps in several areas. There were about a dozen cockroaches in all of them combined which is an improvement. I did not observe any dead rodents in any traps but I did not open any of the enclosed traps. Pest control service invoice dated 12/23/16 was at the restaurant. The worker I spoke with says they have been there two other times since my last re-inspection but the paperwork is not here. 6
99N - THE FEE FOR RE-INSPECTIONS MORE THAN TWICE IN A YEAR ARE 145.00 PER HOUR. 0

6-501.112 - A cardboard and gel trap behind the prep top cooler in the kitchen has about 30 cockroaches on it.. 0
6-501.111(C) - During this re-inspection it is observed that there are still mouse droppings near the steps in the kitchen. It is not entirely clear which are new and which are old. During the re-inspection for mouse droppings I found cockroaches on a flat cardboard/gel trap behind the prep top cooler. The PIC reports that the last visit from the pest control company was November 30th. 6
