99Z - AFTER A DETAILED INSPECTION I SAW AROUND 10 FLIES. THIS IS BETTER THAN LAST TIME. HOWEVER NOT PERFECT. OPERATOR HAS OBTAINED SOME LIGHT TRAPS AND HAD SOMEONE TO COME IN AND WORK ON THIS ISSUE. AGAIN, THINGS ARE BETTER, BUT YOU STILL NEED TO WORK ON THEM. I WILL SIGN OFF ON THIS ISSUE WITH THE AGREEMENT THAT YOU CONTINUE WITH YOUR STEPS TO ADDRESS THE ISSUE. ONE OTHER SUGGESTION THAT I HAVE IS THAT YOU CAULK THE BEER TRAY HOLDER TO THE COUNTER SO THAT NO BEER SPILLS UNDER IT AND PROVIDES FOOD FOR FLIES. 0

THERE IS NO EVIDENCE THAT THE MANAGER HAD THE PEST CONTROL IN TO TREAT FOR THE FLIES, AND AREAS LIKE THE BEER TAP DRAIN TRAY ARE STILL NOT SEALED TO THE COULTER AND CAULK ALL AROUND IT. OPERATOR HAS ONLY INSTALLED ONE LIGHT TRAP FOR THE FLIES IN THE FACILITY. THIS IS NOT ENOUGH TO FIX THE PROBLEM. 3
