The form is not necessary if the County is able to provide the requested information immediately. Many public record requests are made orally and are simultaneously satisfied in the normal course of business; however, if your request cannot be resolved immediately, using the form will help clarify and streamline your request and expedite the proper response from the County. We would also encourage you to discuss your request with the appropriate County department contact person, to clear up any issues so your written request will be asking for exactly what you want.
Page 2 of the Request Form will be used by the County when a request is made in writing, or cannot be resolved immediately. Depending upon the request, the form may have several entries, and you may be required to sign it, before the request is resolved.
