The Oregon Health Authority requires all prospective residential providers to obtain a letter of acknowledgment from the Community Mental Health Program (CMHP).
Use this form to request a Letter of Acknowledgement from Clackamas County Behavioral Health Division. Requests received Monday through Thursday will be processed within 3 business days. Requests are not reviewed on Fridays, Saturdays, Sundays, or County holidays.
A Letter of Acknowledgement will be sent to the provided email address.
Existing residential providers who are making changes to their license (such as adjusting the number of beds) may also use this form.
