The Regional Long-term Rent Assistance (RLRA) program is guided by Housing First principles. Housing First is an approach to quickly and successfully connect households experiencing homelessness to permanent housing without preconditions and barriers to entry. RLRA program partners will work together to preserve participant access to the program and prevent the discontinuation of rent assistance and support services. Every effort is made to avoid a loss of support services, rent assistance, and return to homelessness.
Case conferencing is a collaborative problem solving opportunity and should be an early intervention to avoid loss of supportive services or rent assistance. The service provider, Housing Authority of Clackamas County, or Housing Services Team can initiate the case conferencing process at any time by completing this form. This form can also be used for anyone being supported by SHS case management regardless of voucher type.
Please do not disclose Personally Identifying Information (PII) about the participant in this form (i.e. names, DOB's, address, phone number etc.).
