Primary Care
 

Performance Metrics: Angie Amundson presented the Primary Care Quarter four Performance Metrics. She reviewed the areas they were focusing to improve for 2025. 5 out of 6 metrics were met. 

Four Primary Care Clinic focus areas for 2025: 

1. Well Child Check - Met  
2. Child Immunizations - Met  
3. Depression Screening & Follow up – Met  
4. Diabetes Poor Control – Not met by 2%  
5. BH Integration - Met  
6. Medicare Wellness Visits - Met 

Patient Satisfaction and Experience: Ryan Spiker presented the Patient Satisfactions scores for quarter 4 of 2025. Survey sample size was 166 Surveys. Scores have continued to stay above 90% over the last 8 years with 2024 scoring the highest. 

Overall Satisfaction: Quarter 3 scores have continued to stay above 90% Health Centers is exceeding the National, Regional and State benchmarks. 

Staff worked well together and Loyalty intentions: Both Scores are averaging in the 90% range, scoring over national benchmark 

Patient Satisfaction: Scores saw decreases in a few categories, but the overall scores are still high 

Appointment wait: Score had an increase this quarter. The metric did not meet the benchmarks. This score represents the amount of time a patient had to wait for their appointment from time of scheduling. 

Staff worked well together: Third quarter score increased and is now exceeding the National benchmarks. 

Provider Listening and Wait Scores: Both scores saw an increase and continue to exceed State National and Regional benchmarks. 

Patient Experience: Large increase in the “Waiting area delays communicated” score, which is a positive change. 

Provider Experience: Third quarter increased and is exceeding regional and national benchmark. 

Satisfaction & Outcomes scores were reviewed by Language, Gender, Race and Encounter method were shared. 

Patient comments by type: This data was presented in pie chart form. A chart for Positive Feedback, and Opportunities for Improvement. 153 Positive, 58 Opportunities for improvement

Training and Annual
Disclosure Review
 CHC received training on conflict of interest requirements, including HRSA
Compliance Manual Chapter 13, the Health Centers’ conflict of interest
requirements, and CHC member responsibilities related to disclosure,
abstention, and documentation, along with completing annual conflict of
interest disclosures.

FQHC Staff Report Data Breach: Steve spoke about the recent OCHIN 3rd party data breach.
Health Centers is working with OCHIN to ensure that all effected patients
will be contacted. Health Centers’ staff are being prepared so that they can
assist effected patients with questions.
