Call to Order
Jacqueline called the meeting to order at 5:42 p.m. A quorum was established.
Approval of June 17, 2026 Full Council Minutes
Council Vote
The Council reviewed the minutes for the June 17, 2026 meeting. Jacqueline opened the floor for a motion.
Motion: Brianne motioned to approve the minutes as written.
Second: Debra seconded. No further discussion.
Vote: Approved Unanimously.
Committee Reporting
Finance Committee: Adam shared the year-to-date Revenue and Expenditures report for period ending May 31, 2026.
Adam shared the Special Revenue Addendum as of July 13, 2026.
Performance Metrics and Patient Satisfaction – Primary Care
Primary Care Patient Satisfaction: Ryan Spiker presented Patient Satisfaction scores for Quarter 2. Survey sample size was 165 Surveys.
Overall Satisfaction: Overall score saw a decrease but is still outpacing all benchmarks.
Staff Worked Well Together By Facility: The Clinic locations average score is 91.4%
Patient Loyalty Intentions by Facility: Gladstone received 100% score. Other locations scoring between 95 and 96%
Patient Satisfaction Core: 14 questions in this category with a mean score of 89.5%.
Appointment Wait and Staff Worked Well Together: Appointment wait score currently missing the benchmarks by 2.8%. Staff worked well together and score continues to outpace the National benchmark.
Provider Listening and Provider Wait Scores: Provider Listening Score increased and is now meeting both benchmarks. Provider Wait score saw a decrease. Still surpassing National and Regional but missing the State benchmark by .7%.
Experience Scores: Connected to Care Team Within 15 Mins of App scored 100% while. Response to MyChart Question Within 1 Business Day dropped by 15%
Provider Experience Scores: Provider involved in your care score decreased to 95.4%. No longer meeting the National or Regional benchmark.
Satisfaction & Experience comparisons shared by Language, Ethnicity Gender: Trends reviewed
Patient comments by type: The client feedback data presented in two pie charts. One chart for Positive feedback, and a second for Opportunities for improvement. These charts are broken down into comment themes. 141 Positive comments received, and 48 Opportunities for Improvement.
Primary Care Performance Metrics: Angie presented Primary Care dashboards. Metric goals are set by the Oregon Health Authority.
2026 Metrics:
- Well Child Visits – Meeting quarterly target
- Depression Screening – Meeting quarterly target
- Child Immunization – Meeting quarterly target
- Diabetes Poor Control – Not yet meeting target
- Medicare Wellness Visits – Meeting quarterly target
Health Centers Director Evaluation
Jacque spoke to the results of the Director Evaluation Forms that were completed by the Council. Positive feedback was shared.
Council Education
Adam presented on the 6 W’s of FQHC Wraparound Payments and the Wraparound process.

- Who established the federal mandate
- What is the purpose
- Where do these payments apply
- Why do we need wraparound
In 2025 Health Centers documented 80,466 Medicaid Wraparound Encounters. The Council had an open conversation on the topic.
FQHC Staff Report
Rural Health Transformation: We were not awarded funding for this grant. It was highly competitive, and only one FQHC was awarded in the area.
Strategic Plan Update: Steering Committee was put in place. Consists of implementation teams, which include front line staff as well as leaders. Health Centers is taking a structured approach and will be reporting out on progress to the Council.
National Health Centers Week: Occurs August 3-7. CHC will be included in the communications sent to the staff.
Public Comment
Renel requested that the frequency of in-person meetings be evaluated.
