ADA Accomodation Process

  1. Employee requests accommodation with their supervisor or on Leave Administration website.
  2. Leave Specialist contacts the employee with information and forms to complete.
  3. Forms received, Leave Specialist transitions the claim to Leave Analyst. 
    If no forms are returned within 3 business days, the request will be closed.
  4. Leave Analyst sends Medical Certification to the provider (due within 15 days).
    If Medical Certification is not returned, the claim will be closed.
  5. Leave Analyst reviews Medical Certification and meets with employee and supervisor to facilitate interactive process.
  6. Leave Analyst sends summary response to the request including all offered accommodations.
  7. Implement accommodation or trial accommodation if appropriate.
  8. Review effectiveness of the accommodation with the department and employee.
    • YES. Leave Analyst sends final decision letter and goes to monitoring status
    • NO. Leave analyst and department determine whether there are alternate accommodation available
      • YES Implement alternate accomodation
      • NO Determine if re-assignment is possible
        • YES Re-assignment possible Analyst starts re-assignment process
        • NO No re-assignment possible Starts medical layoff process
