This partnership combines Providence Health Plan’s strong regional provider network and healthcare expertise with Collective Health’s easy-to-use technology platform.
What’s Changing
- New Welcome Packet & ID Cards
Providence Health Plan will mail all members a Welcome Packet that includes new medical ID cards. - New Member Advocate Phone Number (Clackamas County Members)
Members can contact a dedicated Member Advocate team for support at:
866-604-6909 - Updated ID Card Information Required
Members will need to:- Update their healthcare providers
- Update their pharmacy
with their new ID card information once received.
- Collective Health Mobile App
Members are encouraged to download the Collective Health app to:- Access a digital ID card
- Email a copy of their ID card directly to providers
- Manage benefits and claims in one place
