Services for Severely and Persistently Mentally Ill Adults

This program provides comprehensive services for severely and persistently mentally ill adults.

Community Support Team (CST) is designed to assist individuals with stabilizing and controlling their psychiatric symptoms; providing housing assistance; developing a network of support for individuals, their families and friends; and creating opportunities for employment or other meaningful daily activity.

CST services promote independence, productivity and community integration, with our clients being encouraged and supported to be active in decisions and in the development of their own individual treatment plans. The type, intensity and sources of services reflect individual needs and preferences and promote independence. Services are flexible and are primarily provided in our clients' homes and the community. Services available include:

  • Case Management/ Resource Coordination
  • Supported Housing
  • Supported Employment
  • Medication Management/ Medication Skills Training
  • Cognitive-Behavioral Therapy
  • Substance Abuse Treatment
