Health, Housing and Human Services – Health Centers Behavioral Health

Clackamas County’s Health Center’s Behavioral Health Division is hiring a Temporary Case Manager to provide services and support for the Child & Family Treatment Team located at our Milwaukie Lake Road Clinic. This position will be providing case management services to mental health consumers’ ages 12-24. The position includes working in an outpatient clinic, in-home visits, and transporting individuals as part of a service delivery model.
We are looking for an energetic, resourceful, warm and empathic individual who works well independently and as part of a team. The incumbent must have an excellent ability to interact with a diverse client base while maintaining professionalism in a stressful environment. Most competitive candidates will have experience working with the target patient population of youth and emerging adults.
This temporary position is estimated to end in October 2025.
Roles and Responsibilities
- Provide group, individual and family services as appropriate and as identified in the service plan. Actively engage clients in the most appropriate setting (home, office, community). Provide outreach to clients for missed appointments. Use Evidence Based Practices and agency-designated outcome measures to engage clients in monitoring their recovery. Make mandatory reports as defined by state law and agency policy.
- Coordinate with the treatment team to ensure that individuals enrolled in services, and family members as appropriate, participate in assessment and service planning in a timely manner
- Maintain documentation according to agency policy, procedures and standards, insurance requirements, State OARs and Medicaid or Medicare requirements
- Provide case management and care coordination services for youth with Mental Health, Substance Use and/or Co-occurring Disorders. Provide outreach as indicated in clients' treatment and safety plans. Regularly update plans and client eligibility documents. Provide crisis services in the most appropriate settings (home, office, community, virtual). Identify and develop resources and support to enhance client stabilization and well-being.
- Coordinate with health care providers, social service personnel, DHS, Legal Guardians, care home providers, and other agencies to obtain services and coordinate care. Re-evaluate and update client eligibility, advocate for clients with other agencies and service providers. Support clients in attending appointments with other service providers
- Provide individual/family skills training as appropriate and identified on treatment plan on the following topics: safety planning, emotional regulation, health and nutrition practices, housing stability/maintenance, time and money management, building new social networks as a young adult, maintaining family and long term relationships, leisure, coping skills and others as identified
- Conduct comprehensive needs assessments including psychosocial, financial, housing and physical factors. Evaluate functional impairment. Evaluate client Social Determinants of Health through interviews with clients, care givers, guardians and service providers. Update assessments.
- Advocate for clients with family, caregivers, providers, public and private agencies, justice system, schools and other entities as needed. Provide information on clients' disabilities. Respond to emergency and crisis situations in clients' settings. Negotiate problem resolution.
- Perform other duties as assigned
Required Qualifications
- Qualified Mental Health Associate (QMHA) as established by the Oregon Administrative Rule (OAR) or Mental Health and Addiction Counseling Board of Oregon (MHACBO).
- A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
- Bachelor's degree in a behavioral sciences field; or
- A combination of at least three years relevant work, education, training or experience.
- A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
- Must possess and maintain a Basic Life Support (BLS) certificate
- A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
Preferred Qualifications
- Fluency in Spanish/English (bilingual skills to be validated/tested at time of interview and/or selection)
- Experience providing case management services for clients diagnosed with emerging psychosis and/or substance use disorders (SUD)
- Experience working within a multi-disciplinary treatment team
Pre-Employment requirements:

- Must pass a criminal history check which may include national or state fingerprint records check
- Driving is necessary for County business. For position(s) with occasional/incidental driving, incumbents must possess a valid driver's license. Hours and Duration
This position works up to 40 hours during a standard workweek of Monday through Friday.
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
This position may be eligible for a hybrid on-site/telework schedule subject to the Clackamas County Teleworking Policy and based on the Department’s business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during the time of the offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
