Clackamas County Health Centers is seeking a Temporary Case Manager to support the Community Support Team within our Behavioral Health Clinics at our Milwaukie Lake Road Clinic. This is an excellent opportunity to make a meaningful impact by helping individuals experiencing mental health and addiction challenges access the support and care they need to live healthier lives. As part of a multidisciplinary and compassionate team, you’ll play a key role in strengthening our community through direct care and service coordination.
The Case Manager provides care coordination services to adult consumers and, at times, their families, who are living with mental health and/or addiction issues. Many clients served are classified as having severe and persistent conditions. Responsibilities include developing and managing service connections, providing skills training, offering medication support, and ensuring clients are linked with appropriate treatment and recovery resources. Services are delivered within clinics, homes, care facilities, hospitals, and other community settings. This role requires meeting the standards of a Qualified Mental Health Associate (QMHA) as outlined by the Oregon Administrative Rules.
Roles and Responsibilities
- Provide individual, group, and family services based on the client's treatment plan. Engage clients in the best setting for their needs (home, office, or community), follow up on missed appointments, and use evidence-based practices. Track progress using approved tools and making any required reports.
- Offer case management and coordinate services for clients with mental health and/or substance use issues. Connect clients with needed resources, provide outreach, and work with community partners to support care.
- Work with healthcare providers, social services, and other agencies to help clients access services. Provide referrals, regularly review and update care plans, and advocate for clients’ needs.
- Teach life skills to clients and families based on their treatment goals. This includes support with daily living activities like safety planning, communication, managing money and time, healthy relationships, hygiene, and coping strategies.
- Assess clients’ healthcare and social service needs. Interview clients and those involved in their care to understand their challenges and strengths. Complete comprehensive assessments to help guide treatment and support planning.
- Perform other job-related duties as needed.
Required Qualifications
- Qualified Mental Health Associate (QMHA) as established by the Oregon Administrative Rule (OAR) or Mental Health and Addiction Counseling Board of Oregon (MHACBO).
- A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
- Bachelor's degree in a behavioral sciences field; or
- A combination of at least three years relevant work, education, training or experience.
- A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
- A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
- Must possess and maintain a Basic Life Support (BLS) certificate

- Must pass a criminal history check which may include national or state fingerprint records check
- Driving is necessary for County business. For position(s) with occasional/incidental driving, incumbents must possess a valid driver's license. Accommodation requests for an acceptable alternative method of transportation will be reviewed on an individual basis in compliance with State and Federal legislation. Hours and Duration
This position works up to 40.0 hours during a standard workweek of Monday through Friday.
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
This position may be eligible for a hybrid on-site/telework schedule subject to the Clackamas County Teleworking Policy and based on the Department’s business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during the time of the offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
