Clackamas County's Health Centers Division is looking for an experienced Registered Nurse with a desire to work with a medically underserved population by providing comprehensive nursing care to all ages. 

The Primary Care RN leads a team of health care professionals dedicated to providing high quality health care in a team-based care environment. The RN will conduct primary care clinic visits for chronic health conditions, reproductive health, and routine primary care complaints. The RN in this position will provide patient education and ensure clinical competency of Certified Medical Assistants and other members of the care team. RNs are responsible for ongoing management of efficient patient flow, case management, care coordination, as well as leading the team. This position requires independent judgment, effective and respectful communication with patients, ability to prioritize needs, skillful team membership, and expertise to provide nursing care and education. 

The Clackamas Health Centers Division, within the Department of Health, Housing and Human Services, is a Federally Qualified Health Center (FQHC) that offers primary care, dental services and behavioral healthcare at multiple primary care, behavioral health, and school-based health centers. Health Centers serve an ethnically and socioeconomically diverse population. This position provides an opportunity to work with uninsured and underinsured populations with varied needs, ranging from prenatal care to care for the elderly.

This position will be expected to travel as needed between all 4 of our Primary Care Clinics located in Oregon City, Clackamas, Gladstone, and Sandy with the potential to work remotely in telephone triage.

Role and Responsibilities 

  1. Provides direct nursing services to patients; assists individuals and families in carrying out recommendations made by the physician, and in making necessary adjustments in order that the treatment regime can be followed; detects new and early cases of physical and emotional illness, including Social Determinants of health; assists individuals and families to recognize their problems and refers them to the appropriate resources.
  2. Promotes, teaches and demonstrates nursing care to families, individuals, and community groups; provides instruction and information regarding specific diseases or health conditions, as well as health maintenance, and rehabilitation.
  3. Plans and exchanges information with other health workers for effective care of patients; cooperates with other agencies in planning for a patient's continuity of care in hospitals, clinics, schools, industries, homes, and other relevant agencies.
  4. Participates in the planning and operation of clinics by interviewing and counseling patients, performing diagnostic tests, giving prescribed treatment, recording data, making necessary referrals, instructing other nursing personnel and volunteers in their assignments.
  5. May organize, lead, or participate in discussion groups with clients of programs or the agency; compiles material for presentation; prepares teaching aids; performs demonstrations; may assist in the orientation of new personnel.
  6. May participate with local community groups in the development and planning for health services.
  7. Compiles and uses records, reports, and statistical information for appraisal and planning of the assigned duties.

Minimun Qualifications

Any combination of relevant experience, education and/or training that would likely provide the required knowledge and skills to perform the duties of the position.
Possession of a license to practice as a registered professional nurse in the State of Oregon
Basic Life Support (BLS) certification
Must pass a post-offer, pre-employment drug test
Must pass a criminal history check which may include national or state fingerprint records check
Driving may be necessary for County business. For positions with occasional/incidental driving, incumbents must possess a valid driver's license. Accommodation requests for an acceptable alternative method of transportation will be reviewed on an individual basis in compliance with State and Federal legislation.
