Requesting accommodations and auxiliary aids

Individuals supporting a client or patient
Ask the client/patient to contact the county department, clinic, or health care team to ask for accommodation for you.
Filing a grievance
If you believe that H3S has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you may file a grievance with the Civil Rights/ACA Coordinator by phone or through our webform.
