Transportation Reaching People (TRP)

Transportation Reaching People is for Clackamas County residents who are 65+ or have a disability.

The goal of the program is to assist individuals by promoting independent living. Transportation is provided free of charge to medical appointments, grocery shopping and other essential errands.  Donations are welcome!  We can accommodate wheelchair users on most of our vehicles.

TRP qualification and restrictions

  • Must be living in Clackamas County, TRP can provide rides to the surrounding Tri-County areas
  • Participants are 65+ years of age or have a disability
  • This is a curb-to-curb service
  • Rides are not guaranteed but our staff will provide a 3-day advance notice
  • Must be able to independently get on and off the vehicles, and/or with an escort

Clackamas County Developmental Disabilities Resource Fair

The community is invited to the first Clackamas County Developmental Disabilities Resource Fair on Saturday, June 27 from 10 a.m. to 2 p.m. at Clackamas Community College's Gregory Forum Rm G103, 19600 Molalla Ave. in Oregon City

Relatives as Parents Program

Are you the main caregiver for your grandchildren or other relatives?

This program offers support – listening, sharing resources, and connecting you with other programs’ services that support you in your caregiving role. 

Who We Serve

  • Grandparents raising grandchildren
  • Relatives raising children related to them
  • Parents caring for disabled children ages 19 to 59

You qualify if you are:

  • 55 years old or better
  • Raising a child 18 years of age or younger and you are a non-parent relative
  • Caregiving for an adult with a disability and you are their parent or an older relative

Services We Offer

  • Coordination 
    We work with you to apply to programs and for services, including housing, transportation, mental health, physical health care, and developmental disabilities support. 
    We will also work with you to advocate on your family’s behalf with other service providers. 
  • Financial Assistance 
    Grants may be available to help pay for utilities, food, fuel, educational materials, camps, tutoring, counseling, clothing, medical supplies, and furniture. 
  • Listening and Support 
    We host events for relatives as parents and their families to provide connection, encouragement… and fun!
  • Training 
    We provide opportunities for you to learn to take better care of yourself and your loved ones.

Oregon Project Independence

The original Oregon Project Independence Program is closed to new clients at this time.

New Program Option

OPI-M - Oregon Project Independence-Medicaid Expansion

Provides up to 40 hours of in-home help every two weeks depending on your needs. A case manager will support you in coordinating your services.

What's the purpose?

To support people to live at home as independently as possible.

Who is this program for?

People 18 years and older with a physical limitation who need support with things like bathing, dressing, walking, taking medications, transportation, preparing meals, house cleaning and other activities.

How do I qualify?

A case manager will interview you to find out if you qualify based on what you need help with. At the same time, someone will review your income and other finances.

What's the income limit?

Monthly, as a single person, you can have up to $5,320  per month of income which is 400% of the federal poverty level in 2026.

What about my other resources and assets?

  • For 2025, you’re allowed to have up to $99,656 in assets—such as money in a savings account—which is about the cost of six months in a nursing home. This limit is accurate through July 2026, when it will be updated.
  • Your home equity value can be up to $752,000 in 2026.

Am I required to be a citizen of the United States?

Yes, you are required to be a citizen of the U.S. in order to qualify for Oregon Project Independence-Medicaid.

Will the State try to recover the cost of my care after I or my partner die?

No, there is no estate claim for this program.

What is the cost?

There is no cost. The program is free.

Can my spouse be paid to do my caregiving through this program?

No, but other family or friends might be able to provide care for you.

What are other benefits you might qualify for?

  • Some medical equipment

How to apply

Go to your local Aging and People with Disabilities office and ask for an OPI-M application or call the Clackamas County Aging and People with Disabilities screening line and ask for an OPI-M application 971-673-7600.

You can also download the OPI-M application. You can turn in or mail the application to any of the Aging and People with Disabilities offices listed here.

All offices are open Monday to Friday 8 a.m. to 5 p.m.

Medicare and SHIBA Counseling

Are you already on Medicare? Are you new to Medicare?

Open Enrollment is the time to review your Medicare plan, learn what’s new in Medicare, compare and change your Medicare Advantage and Medicare Part D plans or enroll in a plan if you do not already have one. Certified SHIBA (Senior Health Insurance Benefits Assistance) Volunteer Counselors are available to assist with Medicare questions during Open Enrollment and year-round.

Aging Services Advisory Council

Clackamas County Social Services is the Area Agency on Aging (AAA) for Clackamas County.  AAAs are agencies that are designated by the state and federal government to provide services, support and information for older adults and people who experience a disability, so they can live independently and actively engage with their community.

The work of the AAA is supported by the Aging Services Advisory Council (ASAC). Council members participate in the development of planning documents and help the agency evaluate the effectiveness of the programs that are delivered, both by the county and by community partners, including 10 senior centers serving all regions of the county. 

Council Members bring their lived experience and the experience of their family and social networks to inform program development. Members also play a key role in communicating to their networks about information and resources that they learn about via their Council Membership.
 
In 2024, ASAC Members learned about and discussed issues including:

  • Emergency Services in Clackamas County
  • Oregon Project Independence – Medicaid Expansion
  • The Clackamas County Community Action Board Community Needs Assessment
  • Suicide Prevention Efforts in Clackamas County
  • Current State of the Addictions Crisis in Clackamas County

These are some examples of the work that ASAC has done:

  • The ASAC Adult Center Liaison Subcommittee visited local senior centers to review services. They met with the Center Directors and collected data on service provision.
  • ASAC Members provided feedback on the development of a survey tool and process to seek input from older adults in Clackamas County. The survey will inform the County's Area Plan, which addresses the needs of older adults, adults with disabilities, and their caregivers).
  • Representatives from ASAC contribute to the Social Services' Service Equity and Emergency Preparedness Committees and attend the Oregon Association of Area Aging Agencies and Disabilities (O4AD) meeting.

Veterans Advisory Council (VAC)

The Clackamas County Veterans Advisory Council advises the Clackamas County Board of Commissioners and the Department of Health, Housing and Human Services on all matters relating to the provision of services to veterans, their families and dependents, ensuring that the needs of veterans, their families and dependents are recognized and addressed in a timely manner. Veterans Advisory Council also makes recommendations regarding current issues affecting veterans, their families and dependents.
